29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process. IN THE ALTERNATIVE 29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment. (a) On or about February 13, 2018 and at a time prior to, in the 320 Area of the CITGO Refinery in Lake Charles, LA the employer did not implement written procedures to guide the operators while performing tasks that involve assembly of critical bolted flanges in process piping.
