29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: 1) Production Area: On or about February 15, 2018, and at times prior, the employer exposed employees to a struck-by hazard, in that all employees has not received re-certification or evaluation before operating the Mitisubishi forklift truck model number FBC15N and serial A2BS110336, to load and unload materials.
