29 CFR 1910.147(f)(2)(ii): The onsite employer did not ensure that his or her personnel understood and complied with restrictions and prohibitions of the outside employer's energy control procedures: The employer failed to train employees on the restrictions and prohibitions of the outside contractor's lock-out/tag-out (LOTO) program where employees are exposed to amputation and caught-in hazards. This was most recently evident during an inspection on February 14, 2018 at the facility located at 1001 Gallup Dr, Omaha, NE 68102. The company utilizes outside contractors to perform maintenance on various printing and paper cutting machines whereas the contractors utilize their own LOTO procedures. The employer has not trained its employees to understand those LOTO procedures.
