29 CFR 1926.601(b)(4)(i): The employer used motor vehicle equipment that had an obstructed view to the rear, and did not have a reverse signal alarm audible above the surrounding noise level: (a) Employee(s) and contractor employees working / walking on the ground around the Catepillar D250E on the east side of the job site located at 8800 Penrose Lane, Lenexa, Kansas were exposed to crushing hazards due to no functioning backup alarm being available when the dump truck was operating in reverse.
