29 CFR 1910.146(k)(3)(i): Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, or above the entrant's head: a. On or about 1/16/2018 at the establishment located on 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL, employees performing cleaning tasks inside permit-required chemical transporting tanks were not wearing a harness and retrieval line. Employees were exposed to a hazardous atmosphere created by carbon monoxide, in that, an employee lost consciousness inside the tank and had to be carried out by personnel who entered the tank.
