29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about January 26, 2018, employees were exposed to struck-by and amputation hazards when a lockout/tagout program had not been established. a. There was no program describing the employer's implementation of lockout/tagout at the site. b. There were no machine specific lockout/tagout procedures for equipment such as, but not limited to, the chain saw paper cutting machine. Lockout/tagout is required when changing and setting up the saw blade. c. There was no affected employee training for employees that are not authorized to perform lockout/tagout d. There was no authorized employee training for employees that are authorized to perform lockout/tagout e. There was no annual review of the procedures and employees f. There was no program information for how lockout/tagout was to be done by outside vendors who serviced equipment at the site. No abatement certification or documentation is required for this item.
