29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: A) Zehr's Metal & Flat Roofing LLC on or about January 26, 2018 at the address of 7973 Quarry Road, Maumee, OH: The employer did not ensure an employee operating from the Nifty TD34T Aerial Lift utilized a personal fall arrest system that was adequately secured (clipped-off) to the lift's basket. Employees operating from this condition were exposed to falls greater than 10 ft.
