29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator has been involved in an accident or near-miss incident: On or about November 17, 2017, the employer failed to retrain the operator of a forklift who sustained injuries from front loading the equipment and losing the load. The employee came back to work December 7, 2017 and continued to operate forklifts until he was retrained in January 2018.
