29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about January 24, 2018, an employee was conducting maintenance work on the Titan strapping machine and a lock was not applied to isolate the energy source, exposing the employee to the hazard of being caught in the machine.
