29 CFR 1910.146(d)(4)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at no cost to the employees communications equipment necessary for compliance with 29 CFR 1910.146(h)(3) and 29 CFR On December 23, 2017 The employer did not provide adequate communication equipment while employee performed sand blasting in the clarifying tank in that the environment was too loud for the two-way radios provided by the employer to be used by the authorized employee and the attendant.
