29 CFR 1926.502(d): The employer did not ensure that personal fall arrest systems and their use complied with the provisions set forth below. Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. On December 23, 2017 and at times prior, employee(s) were not equipped with a full body harness and exposed to a fall of approximately 40 feet to the concrete below.
