29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Sundial Lodge- Quick Jump Area, 1001 Lafayette Drive, Farmington, PA 15437, On or about December 21, 2017, the CAT Rough Terrain Lift was operated with a fork attachment used for personnel lifting. The equipment's load chart did not reflect the forklifts attachment.
