29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: 1) Production Area: On or about December 6, 2017, and at times prior, the employer exposed employees to struck-by hazards in that an employee operating the Crown truck #SHL 50 model RC3000 and serial 1A212274 was involved in a near miss/accident on November 28, 2017 and was not provided refresher training.
