29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: Production Area: On or about November 07, 2017, an employee was troubleshooting extrusion line #55 and did not use the general procedure established by the employer to lockout the unit before attempting to perform maintenance. This condition exposed the employee to amputation hazards.
