Wilbert, Inc. located in Bellevue, Ohio: On or about November 1, 2017, the employer did not ensure the main electrical disconnect for plastic injection molding press A15 was in working order. The mechanical linkage that disconnects the electrical power inside of the cabinet was missing which required employees to reach into the energized cabinet to disconnect the power.
