Text For Citation: 01 Item/Group: 002 Hazard: FALLING
IC 22-8-1.1 Section 2: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Facility, Lantis K-Loader(s) the chain that was used for safety on the right side of bridge had broken at the weld on -K-Loader Number 32-062 when modified extension broke off and employee fell to his death on or about December 23,2000. "Among other methods one feasible and acceptable abatement method to correct this hazard is to have repairs and modifications to chains non destructive tested to determine the rated capacity as specified in 29CFR1910.23(e)(9)". (b) Facility, K-Loader(s) Welds had been damaged and parts re-welded on as well as the welds on the modification of the Lantis Model 818-144 in which one weld broke off the bridge on the right side causing an employee to fall to his death on or about December 23, 2000. " Among other one feasible and acceptable abatement method to correct this hazard is have non-destructive test or x-ray by an approved entity."
