Text For Citation: 01 Item/Group: 002 Hazard: FALLING

Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: At the combustible tanks located in the Thermoelectric Central in Palo Seco, employees had to take measurements of the combustible level in the measurement point located in the roof of the tanks. The thickness inspections of the shell were not made at the time intervals specified in the API standard Sections 4.3.3.2 a that state "When the corrosion rate is not know, the maximun interval shall be five years", or API section 4.3.3.2 b that state "When the corrotion rate is kown, the maximun interval shall be smaller of RCA/2N or 15 years". The tanks with this violation, were the following: a. Tank R-1 b. Tank R-2 c. Tank R-3 d. Tank R-4 e. Tank S-1 f. Tank S-2 g. Tank S-3 h. Tank S-4 Hazard: Collapse of roof plates and fall into the tank. Among other methods, one feasible and acceptable method to correct this hazard is to follow the instructions specified in the standard API 653, Sections 4.3.3.2 a and 4.3.3.2 b. En los tanques de combustible localizados en la Central Termoelectrica de Palo Seco, empleados tenian que tomar lecturas del nivel del combustible en el punto de muestreo del techo del tanque. Las inspecciones de espesor no se realizaron en los intervalos de tiempo especificados en el estandar API 653, Seccion 4.3.3.2 a que establece "Cuando no se conozca la razon de corrosion, el intervalo maximo tiene que ser de 5 anos o la Seccion 4.3.3.2 b que establece: "Cuando la razon de corrosion se conozca, el intervalo maximo tiene que ser menor de RCA/2N o 15 anos. Los tanques con esta violacion fueron los siguientes: a. Tanque R-1 b. Tanque R-2 c. Tanque R-3 c. Tanque R-4d. Tanque S-1 e. Tanque S-2 f. Tanque S-3 g. Tanque S-4 Riesgo: Colapso de planchas del techo y caer dentro del tanque. Entre otros metodos, un metodo factible y aceptable para corregir este riesgo, es seguir las instrucciones especificadas en los estandares API 653, Secciones 4.3.3.2 a y 4.3.3.2 b.3.3.2
