29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) 899 Route 22, Brewster, NY, 12205 - On or about 10/4/17, employees were installing the mast sections on a mast climbing scaffold and were exposed to fall hazards of approximately 12 feet. The employees were not protected from falls by use of a conventional fall protection system. The employer did not develop and implement a training program for all employees on the recognition, evaluation, and control of hazards associated with scaffolding operations, including, but not limited to, fall hazards.
