Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices shall be substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: On or about 05 October 2017, the employer did not ensure that lockout devices (locks) applied by authorized employees provided substantial protection against removal during servicing and maintenance. All Lockout locks were keyed alike and the supervisors also had keys to unlock the devices. This exposed employees to amputation and/or crushing injuries from unexpected start-up. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
