29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A) Uni-Grip, Inc., located at 9823 State Highway 53 North, Upper Sandusky, OH 43351: On or about September 21, 2017, the employer did not ensure that the point of operation of the equipment such as, but not limited to, the hole puncher machine (built in-house) was adequately guarded. Employees were exposed to amputation hazards while operating the machine.
