On or about September 25, 2017 located at 11332 Tamarco Dr. Blue Ash OH along interior non-leading edge areas of the structural steel structure employees performed metal decking dismantling associated activities without maintaining structural stability of the metal decking exposing employees to inadequately supported structural metal decking collapse or failure fall hazards in excess of 21 feet.
