29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Building 25024, Andersen AFB: A worker on the roof installing guardrail components had his lanyard snaphook attached to the positioning device side dee-ring at waist level. The employee was exposed to fall hazards of up to 108 inches.
