29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): 1. On or about August 28, 2017, maintenance employees performing unjamming activities at the oversized culling drum were not required to utilize Specific Energy Control application steps for the Advanced Facing Canceller System (AFCS 200) machine for energy isolation. Maintenance employee required to climb into the oversized culling drum area to clear jams are exposed to the hazard of being caught in hazards. 2. On or about August 28, 2017, AFCS 200 operators performing unjamming activities at the reject chute near the oversized culling drum were not required to utilize Specific Energy Control application steps for the Advanced Facing Canceller System (AFCS 200) machine for energy isolation. Operators clear jams by hand at the reject chute located right next to the oversized culling drum and are exposed to the hazard of being caught in hazards. 3. On or about September 11, 2017, maintenance employee putting the drive belt back on inside the oversized culling drum were not utilizing Specific Energy Control application steps for the Advanced Facing Canceller System (AFCS 200) machine #1 for energy isolation. Maintenance employee required to climb into the oversized culling drum area to get access to the drive belt are exposed to the hazard of being caught in hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. The U.S. Postal Service a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1229207, citation number 1, item number 2b and was affirmed as a final order on October 4, 2017, with respect to a workplace located at 1025 West 20th Street, Oshkosh, WI 54902.
