Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Plant Floor/PDC Tamper Bander Machine: On or about August 21, 2017 and at times prior, the employer exposed employees to amputation hazards, in that employees were allowed to perform set-up, maintenance, and clearing operations on equipment such as, but not limited to, the PDC Tamper Bander machine, without developing, documenting and utilizing lockout/tagout energy control procedures to prevent accidental startup of the machine. Employees were allowed to clear jams and clean the machine using only the E-stop and/or photo eyes in lieu of locking out the machine.
