29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Employees were exposed to amputation, crushed-by, and struck-by hazards. The employer had not provided training for authorized or affected employees regarding energy isolation or lock out tag out procedures. Feed Mill CDL drivers were required to perform service or maintenance on equipment such as but not limited to the Mega Flow Pro Auger serial number 858.
