29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) The employer did not ensure employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or flying objects. This violation occurred on or about August 2, 2017, and at times prior thereto; when employees were working in the grinding operation area where there was a potentially struck by flying debris from grinder machine, were not wearing head protection.
