29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On 8/1/17, at the Gainesville, Georgia facility, in the engine room, the employer did not ensure walking surfaces were repaired or covered where equipment had been removed. Pits measuring 21 inches deep and 2 feet wide were left exposed.
