29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Dover Christian Church, 1738 Forrest Avenue, Dover, DE - On or about July 31, 2017, the employer failed to ensure that subcontractor employees used fall protection while working at a height of approximately 11 feet above grade. On July 31, 2017, a Menno's Construction employee fell to his death while setting trusses.
