29 CFR 1910.30(a)(1): Before any employee was exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart: a) 1761 Route 9 North, Howell, NJ, 07731- Blend Mixer loading platform Northwest corner: Employees were exposed to a fall of approximately 8 feet to the ground below and a hole of approximately 6 inches when working on an elevated platform, occurred on or about 6/21/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
