29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Perry Tool & Die Co. at 11606 Reed Ct., Broomfield, CO 80020: On and before June 23, 2017 the employer did not ensure the points of operation were properly guarded to prevent employees from having any part of their body in the danger zone during operation. This condition exposed the employees to crushing and amputation hazards. This condition includes but is not limited to: (1) Amada press brake model RG-50, S/N 508920 Unguarded point of operation presented crushing and amputation hazards. (2) Amada press brake model RG-100, S/N 103634 Unguarded point of operation presented crushing and amputation hazards. (3) Amada press brake model RG-35S, S/N unknown Unguarded point of operation presented crushing and amputation hazards. (4) Amada press brake model RG3512LD, S/N 35120135 Unguarded point of operation presented crushing and amputation hazards. (5) Rother Machine Tools hydraulic press brake S/N 10VSG/A2066 Unguarded point of operation presented crushing and amputation hazards. Abatement Note: Press brake safeguarding can be accomplished by using one of the following: (a) Presence Sensing Device - Designed to automatically stop the machine when the sensing field is interrupted. (b) Two-Hand Control Device - Designed to keep the operators hands away from the point of operation by requiring two hands to engage the machine. (c) Pullback Device - Designed to pull the operator's hands away from the area of the closing dies during each stroke of the press. (d) Restraint Device - Designed and adjusted so that the operator can never reach the point of operation.
