29 CFR 1926.451(b)(4): Each end of a platform, unless cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches: (a) On or about June 17, 2017, at a jobsite located at 7950 Ohio Drive, Plano, TX, employees using a 2-inch hose to pump concrete into the holes of a concrete wall while working from a pump jack scaffold platform with unsecured planks were exposed to a fall hazard.
