29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about June 2, 2017, at the Acrylonitrile Unit, the employer failed to ensure that an authorized employee adequately affixed lockout or tagout devices on the isolation points of the acid system to prevent unexpected operation before allowing contract workers to begin line-breaking operations.
