29 CFR 1910.140(c)(18): Personal fall protection systems were not inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components were not removed from service: a) Warehouse: A 3M Safelight LOPRO Shock Lanyard model 209712 used with order pickers, including, but not limited to a Crown SP 3000 series vertical upright forklift, had a pull in the polyester webbing and was not removed from service, on or about 6/21/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
