29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: (a) Schmitt Construction LLC., at 303 Emmett Trail Harrisburg SD, 57032: On and before June 1, 2017, the exposing employer did not ensure employees were protected from fall hazards associated with using an off road forklift as a means of mobile scaffolding. Employees placed a piece of plywood on the forks of the off-road forklift and as a result of doing so, resulted with an employee falling from the platform.
