Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) being struck by or crushed by falling parts of elevated hoist mechanisms, such as, but not limited to traveling block, hook and pulling line, oil rig pulling line, four parts on Pemco derrick , s/n 1119, model #96-180-c, rig #435. Three wire rope clips were used to secure the dead end connections of pulling lines to a flat plate about 4" x 10" x 1". The U-bolts had gone through a forceful change (cut-into), and welded to each side of the steel plate. Re-used wire rope clips were improperly installed and were not capable of being torqued to its full recommended value. Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with "The Crosby Group Product Warning", Edition 7, Technical Data Sheet (re-use of wire rope clips) 08/14/92.
