29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Concrete crushing area: On or about May 2, 2017, employees engaged in crushing concrete worked around and operated a rock crusher were not wearing available hard hats. Concrete pieces fell from the crusher sides exposing the employees to struck by hazards.
