29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about 1 May 2017 and at times prior thereto, specific lockout/tag out procedures were not developed for machines with more than one energy source, exposing employees to the hazard of being caught in or struck by moving machinery parts. A) Large Gluer- powered by electric and pneumatic power, no specific LOTO procedures. B) Jumbo Slotter/Printer- powered by electric, pneumatic and hydraulic power, no specific LOTO procedures.
