29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer is failing to protect employees from struck-by hazards associated with maintaining equipment without having developed or implemented specific written procedures for securing stored energy. The most recent occurrence was evident during the inspection conducted on May 4, 2017 at the facility located at 13433 Centech Rd, Omaha, NE 68138. The employer has employees changing blades on the Hem and Do-All saws without having developed and implemented written procedures that specifically identify the steps necessary to control the release of stored energy.
