29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a.) On or about May 3, 2017, the employer did not ensure modifications such as a homemade working platform used on the Hyster LP forklift was used with manufacturer's approval. Employees are required to use the working platform to perform tooling changes and support the head tools, and are exposed to hazards associated with powered industrial vehicles.
