29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about April 25, 2017, at the steel distribution and fabrication facility located at 2400 N.E. 36th Street, Fort Worth, Texas the employer did not ensure employees were provided with fall protection exposing employees to fall hazards of approximately (8) feet to the ground below. 1. Shipping and receiving employees loading and unloading trucks were exposed to fall hazards. 2. The break press upper level platform was not equipped with guard rails exposing maintenance employees to fall hazards.
