29 CFR 1910.28(b)(3)(iv): Each employee was not protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used: a) On or about 4/24/2017 and at times prior thereto, employees walking and working in the northwest corner of the grain cleaning room, in the vicinity of the ladderway floor hole to the emergency ladder that was not provided with a guardrail system and toeboards, were exposed to a fall hazard.
