Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of not being able to control the well during a kick or blowout due to the failure of Farrar Oil Field Service & Equipment Co., Inc.'s crew of workover rig #4 to secure the well at the first sign of intermittent or uncontrolled eruptions of gas and/or fluids from the R. & H. -Travis #3 Well located in Section 6, TION, RPW, Jasper County, MS. This occurred on or about Tuesday, May 29, 1990. Among other methods, one feasible and acceptable abatement method to correct this hazard would be the following sequence of steps being taken at the first sign of kick or threatened blowout: 1) Stab (screw in) a full-open T.I.W. valve, with gauge, in the tubing string. 2) Close tubing T.I.W. valve. 3) Close rams of one or more blowout preventers. 4) Read and record shut-in pressures on the tubing and casing sides of well. 5) Initiate the approved well kill/control method, as specified in Section 8.2, page 35 of A.P.I. RP-59, Recommended Practices for Well Control Operations, First Edition, 8/20/87.
