29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph: The following are examples of hazards not addressed in the Process Hazard Analysis: (a) Manipulating and transferring a full cloth of dried (Tetrazene) within the muslin cloth from the vacuum receiver to the weighing table without knowing the moisture content of the Tetrazene in . Transferring Tetrazene from the vacuum receiver to the weighing table without knowing whether the Tetrazene is within safe handling limits exposes high explosive mixers to explosion hazards. (b) The wedging action (using a spatula to break up the tetrazene cake in the dryer) utilized in the Tetrazene de-watering process was not addressed in the Process Hazard Analysis.
