29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) STTA In-Line Machining Centers, Production Floor: On or about March 28, 2017 and at times prior, employees were exposed to caught-in, struck-by, and crushed-by hazards, when performing servicing, setup, and maintenance activities on equipment including, but not limited to, STTA; A3; A4 & A6 In-Line Machining Centers (1-21) which had hydraulic, pneumatic and electrical energy sources, in that each authorized employee was not provided with locks for isolating and securing machine energy sources.
