Text For Citation: 01 Item/Group: 002 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazard of splash back of chemicals such as chromic acid, nitric acid, hot water from loss of load due tohoist failure in that the overhead hoists in the Brazing, Anodize and Alodize departments were not properly maintained in that in the following areas; oThe hand operated gear hoist located in the Alodize department did not have the rated load capacity marked on the hoist or the load block, was not inspected on a frequent and periodic basis, had no preventative maintenance program oThe air operated hoist located in the Anodize department did not have the rated load marked on the hoist or the load block, the controls for the air hoist created a hazard in that they were on a separate bridge than the hoist itself, exposing the airlines to potential hazard of tanglement, damage and/or disconnection, was not inspected on a frequent and periodic basis for the hoist and auxiliary equipment, no preventative maintenance program was established, the hook latche was missing, and the pendant controls were not legibily marked as to their function. oThe below the hook lifting attachments such as but not limited to; the baskets and the racks used departments such as but not limited to the Brazing, Anodize and Alodize departments were not marked as to rated capacity, had no engineering data available to show that they were designed to withstand the forces imposed by its rated load, with a minimum design factor of 3, based on yield strength and had not been proof tested with a written report prior to use Among other methods feasible and acceptable abatement methods, to correct these hazards is to test the below the hook lifting attachments, develop and implement hoist inspection and maintenance procedures in accordance with ASME B30.16-1993 for Overhead Hoist (underhung), ANSI B30.10-1993 for Hooks and ASME B30.20-1993 for Below-the-Hook Lifting Devices for all hoists, hooks and below the hook attachments such as the baskets and racks.
