29 CFR 1910.28(b)(3)(i): Holes. The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: Employees were exposed to a fall hazard. On or about March 5, 2017, employees working in the Stage 3 Truck E-coat pre-treatment area, were exposed to fall hazards when performing scheduled routine maintenance on the spray nozzles. Employees were not protected from falling through a hole in the platform that was 4 feet (1.2 m) or more above a lower level by way of cover or fall protection.
