29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: On or about 03/13/2017, at 2021 Tara Blvd, Baton Rouge, LA, employees were conducting sewer work in a trench with heavy equipment at rest on the immediate edge of the excavation. GRADY CRAWFORD CONSTRUCTION CO., INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH IS CONTAINED IN OSHA INSPECTION # 566838, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED A FINAL ORDER ON 9/26/12, WITH RESPECT TO A WORKPLACE LOCATED AT CORNER OF PREJEAN AND HWY 78, LIVONIA, LA.
