29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, first floor, the batch mill line #1 operator was exposed to noise at a time-weighted average of 99 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. b) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, first floor, the batch mill line #3 operator was exposed to noise at a time-weighted average of 97.6 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. c) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, first floor, the TSR operator was exposed to noise at a time-weighted average of 94.6 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. d) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, second floor, the mixer #2 operator was exposed to noise at a time-weighted average of 93.1 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. e) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, second floor, the mixer #4 operator was exposed to noise at a time-weighted average of 92.7 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. f) Kumho Tire Georgia, Inc., Macon, GA - In mixing building, second floor, the mixer #3 operator was exposed to noise at a time-weighted average of 91.7 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. g) Kumho Tire Georgia, Inc., Macon, GA - In tire inspection area, the TUG line #1 operator was exposed to noise at a time-weighted average of 86.5 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA. An audiometric testing program had not been established, on or about 3/8/17. h) Kumho Tire Georgia, Inc., Macon, GA - In tire inspection area, the TUG line #1 temporary employee was exposed to noise at a time-weighted average of 85.8 dBA, which exceeded the 12 hours shift extended hours action level of 82.1 dBA.
