29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: a) North side of entrance overhang: Employees without any fall protection worked on the underside of the entrance overhang from the makeshift scaffold fabricated from left over lumber at heights between approximately 9 feet 10 inches and 11 feet, on or about 02/24/2017; b) North side of entrance overhang: Employee without any fall protection approximately 18-20 feet above ground level, without any type of fall protection, was standing on top of an unsecured sheet of OSB resting on the forks of the Model B-66 Pettibone Telehandler while removing construction debris from the roof of the overhang, on or about 02/254/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
