29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: a) On or about 3/7/17, the R&D Chiller Chemical Room Unit 24 P&ID (Drawing 13115R-6C) Surge Drum pressure relief valves was missing the existing header piping section. b) On or about 3/7/17, the R&D Chiller Chemical Room Unit 24 P&ID (Drawing 13115R-6C) indicated .75 inch Surge Drum HTS pipe line connected to the 2.5 HTS pipe section when it physically connected to the 3 inch HTS pipe section. c) On or about 3/7/17, the Silo 6O01 P&ID (Drawing 13115R-6N) indicated the 1.25 inch HTRS pipe section connected to Silo #29 with a single pipe connection when physically the 1.25 inch HTRS pipe section branched into 3 connections with Silo #29. d) On or about 3/7/17, the Recirculator and Transfer P&ID (Drawing 13115R-4B) indicated 4 pipe sections connected to the Recirculator 2 inch HTRS pipe section containing hand valve 4B01 in the following order: nearest to Recirculator is 1 inch HTRS pipe section containing Hand Valve 4B02, 1 inch HTRS pipe section containing Hand Valve 4B03, 1.25 HTRS pipe section containing hand valve 4B58 and nearest the 2 inch HTRS pipe section containing hand valve 4B01 capped end is .75 inch HTRS pipe section containing hand valve 4B47. The physical order of the pipe sections connecting to the 2 inch HTRS pipe section containing hand valve 4B01 is: nearest to the Recirculator is the 1.25 HTRS pipe section containing hand valve 4B58, 1 inch HTRS pipe section containing Hand Valve 4B02, .75 inch HTRS pipe section containing hand valve 4B47 and nearest the 2 inch HTRS pipe section containing hand valve 4B01 capped end is 1 inch HTRS pipe section containing Hand Valve 4B03.
